Custom Order Forms
We are providing Custom order forms to make ordering of custom uniforms easy. Download the order form, fill it out and send it to us. You can either email the completed form or print it out and fax it to us. The email address and fax number are provided on the form. For your convenience, we have provided the form in Microsoft Excel format and PDF format. If you do not have Microsoft Excel, please use the form in PDF format. If you don't have Adobe PDF Reader, click on the "Download Adobe Acrobat PDF Reader" link below to go to Adobe's website and then download the reader from there.
After we receive the completed order, an email confirmation will be sent in 1-2 business days. This email will contain the details of your order and the total cost. We will only contact you if we have any questions about your order. Otherwise, the order will be processed. So, please send us the completed form only when you are ready to place your final order and make sure that the information provided on the order form is accurate. Greatvalueuniforms will not be responsible for any mistakes on the completed order form.
